    I certify that the information I provided above is true, correct, and complete. I understand that scholarship recipients are selected by committee and approved by Mr. Kortney Grant in his sole discretion, and that the $5,000 award will be paid directly to my educational institution to be applied towards my tuition, fees, and books. I further understand that the award will not be paid to me individually and may not be used for any purpose other than as set forth herein.

    If selected for the scholarship, I hereby grant 3C the right, license, privilege, and permission, to take and record photographs, audio recordings, and video of me, including, without limitation, my image, name, voice, likeness and/or other identifying attributes, characteristics and/or features of me (the “Materials”) and to use, publish, publicly display, distribute, reproduce, alter, broadcast, transmit, stream, make available for download, such Materials for any lawful use 3C may deem proper[cite: 1].
